CommuniCare Health Centers (CCHC) is a Federally Qualified Health Center providing health care since 1972 to anyone regardless of insurance status, immigration status, or the ability pay. Using the patient-centered medical home model of care, CCHC puts patients first by providing comprehensive quality health care services delivered by a dedicated team of providers and support staff.
